In consideration of participating in the curated experience coordinated by THE DATING BOX COMPANY LLC, an Oregon limited liability company (“Company”), I acknowledge and agree to the following:
Voluntary Participation
My participation is voluntary. I may decline to answer any questions in the planning questionnaire or withdraw from participation at any time.
No Medical Advice
I understand that Company is not a healthcare provider and does not provide medical advice. I am responsible for consulting my own physician regarding any health or medical concerns.
Health & Food Allergies
I have disclosed any known allergies or restrictions. Company will make reasonable efforts to accommodate this information but cannot guarantee an allergen-free environment. I remain solely responsible for managing my own medical needs, including carrying medications such as epinephrine.
Risks of Participation
I understand that certain activities may involve inherent risks, including but not limited to travel, dining, and physical exertion. I voluntarily assume all such risks.
Third-Party Providers
I understand that some services may be provided by third-party vendors, and Company is not responsible for their acts, omissions, or negligence.
Limitation of Liability
To the fullest extent permitted by law, I release and hold harmless Company, its officers, employees, agents, and contractors from any and all claims, demands, damages, or causes of action arising out of or related to my participation, except to the extent caused by Company’s gross negligence or willful misconduct.
Eligibility & Age Requirement
I represent and warrant that I am at least 18 years of age and legally competent to sign this Waiver.
